Quality Assurance. All service users
attending Hayfield Support Services on a residential and / or day care basis
will have an individual assessment and care plan. Each individual
assessment and care plan will include the views of service users, carers and the
referring authorities, as well as the contributions of day care and residential
staff. Where appropriate,
individuals will be encouraged to make use of external, specialist services
(psychiatric / psychological services, educational services, vocational training,
etc.). External contributors will
have the opportunity to have their views included in care planning. Service users will
have the opportunity to seek the advice and support of independent advocacy. Service users will
have the organisation’s “Complaints Procedure” available to them in an
appropriate medium. Prior to admission to
Hayfield Support Services all service users, their carers and referring
authorities will have access to detailed information about the services
available and to the Policies and Procedures which operate. Each applicant will
initially undertake a 6 week residential and day care assessment.
The results of this assessment will be recorded, and the contents of the
subsequent assessment report will be made available to the applicant, their
carers and their referring authority. Full admission to
Hayfield Support Services will only be arranged
when such admission is deemed appropriate, and when agreed to by the
applicant, their carers and the referring authority. Subsequent to
admission, the progress of each
individual service user will be monitored and recorded. Each service user
attending Hayfield Support Services will be assigned to a named residential key
worker, and named day care instructor. The situation of each
service user will be fully re-assessed on a twice yearly basis.
Full written reports by the key-worker and instructor, and any other
appropriate information, will be presented at regular review meetings, which
will be open to the service user their carers and representatives of the
referring authority, and any other appropriate contributors. Services provided to
each service user may be rearranged or altered, as deemed appropriate, by
discussion and agreement through the review system. Service users resident
in Hayfield Support Services are free to receive visits from carers or
representatives of their referring authority at any time. Service users or their appropriate representatives will have a right of access to written reports and information about that service user, as detailed under the “Access to Information” legislation. Hayfield Support
Services are inspected and registered by the Care Commission. Copies of the most recent 'Inspection Report' can be made
available to referring authorities, on request. Service users and
their representatives will have access to Hayfields “Complaints
Procedure” on request. Hayfield Support Services will endeavour to
recruit staff with adequate and appropriate skills to meet the needs of the
organisation and the service users, or
to provide the necessary training to achieve
these skills. All staff will receive regular supervision by their line manager, and will attend regular staff meetings.
