Outcome Standards.

Deaf individuals with additional difficulties are referred to Hayfield Support Services by a range of statutory authorities.  Prior to referral / admission there is an acknowledgement that each individual has specific special needs that have to be addressed and provided for.

Referral to Hayfield is followed by a six-week day care and / or residential assessment, which seeks to complement the initial referral criteria by identifying a number of specific areas of need, and subsequently setting a range of practical, social and / or emotional goals for that individual.

At the end of the assessment period, a meeting is held at which all relevant participants, including Hayfield staff, the service user, carer / family, and referring authority worker can decide on whether a placement in hayfield is appropriate and, if necessary, set specific goals for the next review period.

All Hayfield service users will have their continued placement reviewed twice yearly and there will be an assessment as to how far they have progressed in achieving previously set goals, and where appropriate new goals will be set.

The initial assessment and all subsequent reviews will consider a number of key areas for personal development for each individual service user, as follows.

Outcome Standards...

  • Service users will live in an environment where there are opportunities for fun and pleasure.  The environment will be safe and suitable for their individual needs.

  • Service users will have opportunities to discuss their interests; skills and abilities with staff and this information will be used appropriately.

  • Service users will be able to maintain existing contacts and relationships and to make new ones

  • Service users will have access to information about activities and amenities in and outwith the establishment, communicated to them in an appropriate medium.

  • Service users will be encouraged in all aspects of their education, vocational training, employment or occupational interests, as identified in their individual care plan.

  • Service users will be able to participate in activities associated with their social, recreational, spiritual, and cultural interests, with appropriate supports as required.

  • Service users will be encouraged to make use of external educational or training programmes, with support if necessary.

  • Service users’ personal hobbies or interests will be encouraged by the involvement of external groups or individuals, with appropriate supports.
