Monitoring
Outcomes - External All service users are
referred to Hayfield Support Services by external statutory authorities.
These authorities establish the initial criteria of need for each
individual, and will subsequently be responsible for continued involvement with
the service user through financial obligation and through participation in the
ongoing review process. Where appropriate
service users are encouraged to maintain contact with previous carers / family,
who may participate in the ongoing review and decision-making process. Social workers or
other representatives of the referring authority are requested to maintain
contact with the service user throughout the period of the placement.
They are expected to produce all relevant information regarding the
service user’s personal and family background, and to be involved in the
ongoing monitoring process. This
involves attendance and participation in the regular review process, as well as
giving the service user opportunities to discuss their placement, expectations
and future goals. Service users are also
entitled to seek the help and support of independent advocates, who may attend
review meetings and have access to information about the service user with their
agreement and within the limits of the “Access to Information” Legislation. During their placement
within Hayfield, service users have the opportunity to have contact with a range
of external services, depending on their individual needs, e.g. psychiatrists,
counsellors, therapists etc. Where appropriate and relevant, these other agencies can be
involved in ongoing assessment and future planning. Referrals to such external services will be notified to the
responsible referring authority.
