Service User's Agreement

This agreement is made between Hayfield, 260 Moffat Street, Glasgow, G5 0ND, and the Service User, or their representative. 

This agreement allows the Service User to live in Hayfield’s residences, in a single room and attend Hayfield’s Day Care Services.

  • The Service User (or his / her representative) will pay to Hayfield a weekly charge.  The exact amount will be calculated after your specific needs are known. 

  • The weekly charge to the Service User may be altered annually but there will normally be no more than one change each year.

 

Allocation

  • The Service User will be allocated a single room in one of the following residences:
  • 268 Ballater Street, Glasgow, G5 0YT.

  • 4/3 Buccleuch Street, Cowcaddens, Glasgow, G3 6SL.

  • 4/4 Buccleuch Street, Cowcaddens, Glasgow, G3 6SL.

  • 106 Drumover Drive, Tollcross, Glasgow, G31 5RP.

  • Hayfield reserve the right to move the Service User between residences in keeping with the developmental nature of the service.

  • The Service User will be given reasonable notice and discussions will occur before such a change is made.

  • In addition to the single room, the Service User will also share various communal facilities with other Service Users in the residence.

  • Hayfield will provide care, support or habilitation to address the individual Service User’s personal and special needs.  Details of the programme will be reviewed regularly by both staff and the Service User and adapted, if necessary, to meet his / her changing needs.  A review will be held twice a year where the Service User and his / her significant others attend, e.g. Social Worker, Family Member, etc. 

  • The Service User agrees to give staff access to the single room they are occupying in order to provide the care, support or advice described previously.

 

Maintenance

Hayfield’s Responsibilities:

  • Hayfield is responsible for ensuring the property is maintained in good repair, both inside and out.

  • Hayfield will provide and maintain suitable furniture and fittings.

  • Hayfield will keep the property fully insured against loss or damage caused by fire and other risks covered by normal building insurance.  Service Users personal effects are insured by Hayfield up to a limit of £1,000.  However, any individual items of value should be insured by the service user separately.

The Service User’s Responsibilities:

  • The Service User must notify Hayfield promptly of any repairs which are necessary and agree to allow the staff, or workmen instructed by Hayfield, access to the property/room to carry out these or any other repairs.  Hayfield will, whenever possible, give the Service User notice of access being required but, in an emergency, immediate access may be necessary. 

  • The Service User, subject to individual ability, is responsible for ensuring that his / her single room is kept in good order, clean and tidy and is also responsible, where appropriate, together with other residents, for looking after the communal areas of the property.  The Service User further agrees to take part in fire drills, to attend meetings as required, and to obtain written permission from Hayfield before (a) keeping pets (cats, dogs and large pets are not allowed), and (b) making alterations to fixtures or fittings. 

NB Hayfield reserve the right to refuse a pet for a Service User if that Service User cannot care for the pet properly. 

  • The Service User agrees to meet the cost of repair or replacement of items damaged through neglect or carelessness on the part of the Service User or any visitor visiting him/her. 

  • The Service User must not allow anyone else to occupy or share the accommodation allocated under this Agreement without exceptional formal permission by Hayfield. 

  • The Service User must not obstruct their bedroom door, change locks

  • or add internal bolts for Health & Safety reasons. 

  • The Service User agrees not to cause or permit any visitors to cause any disturbance or nuisance within the residence that is either in the single room or the communal areas or in the vicinity of the property. 

  • The Service User further agrees not to smoke in their bed/bedroom but only to smoke in the designated smoking area. 

  • The Service User agrees that he / she must not act in a way that is disruptive to any other Service User, their visitors or staff.  The Service User agrees not to do anything to affect the safety and quiet enjoyment of the residence by other Service Users. 

  • The Service User is expected to work positively and co-operatively with their agreed Care Plan and the staff involved. 

  • The Service User agrees not to threaten violence, not to behave aggressively to other residents or staff and not to bring any offensive weapons or prohibited drugs into the property.  Excessive consumption of alcohol is not allowed in the property (See Alcohol Policy).

 

Temporary Absence

  • If you are away from Hayfield residential setting for the following reasons, for short periods of time, your placement will continue.
  1. Holiday periods.

  2. Hospitalisation.

  • You will require to consult with your social worker about extended periods of leave, in order to establish that the Social Work Department/Social Security is willing to continue payment of your placement fee and that DSS arrangements are in order.

 

This Agreement Can Be Ended By

  • The Service User or his / her representative giving 4 weeks written notice of his / her intention to leave. 

  • The Service User and Hayfield agreeing that the care, support or habilitation being provided by Hayfield as part of the Agreement is no longer appropriate or required.  When this is the case, Hayfield will inform the appropriate agencies, e.g. Social Work, who will assist the Service User to find alternative suitable accommodation.                     

  • The Service User leaving the single room vacant for 7 days or more without informing Hayfield of his / her absence. 

  • Hayfield giving the Service User notice that he / she is required to leave the single room and turn over the keys to staff. 

  • The Service User knowingly causing distress or danger to other occupants, staff or the structure of the building and not adhering to any or all of the terms of the Service Contract.  In the event of such a situation occurring, the Service User may be required to leave immediately.

  • The Service User reaching the pre-arranged demission date.

 

Complaints

  • In the event of the Service User being dissatisfied with the operation of this agreement, he / she is entitled to complain using the procedures set out in Hayfield’s Complaints Procedure.
