Referrals |

Prospective male and female applicants should be 16 years and over. Referrals are generally made by social workers, but can come via hospitals or by self-referral. The decision to accept a particular referral is made by a panel of senior staff after careful consideration of all relevant documentation. The referral process is outlined on the following documents:
All candidates are invited for
a six-week assessment. This enables
staff in both the residential and day care sections, to work with the
prospective applicant and make a detailed assessment and care plan with
extensive recommendations. The six
week period has a dual purpose, as it also allows the prospective service user
to get to know what is available and to get a feel for whether or not they would
be able to comfortably settle down and gain from such a placement.
The prospective candidate, referring agency and assessment staff meet
together toward the end of the six week assessment and decide the way forward.
Detailed documentation is completed, outlining the findings of the
assessment period. If the prospective applicant decides to take up the place offered, they are given a Conditions of Stay document. This
outlines what Hayfield offers, the expectations from the individual and gives
information to help the new service user settle into their surroundings. If required, staff explain the contents of such written
material in British Sign Language (BSL) and other non-verbal / visual mediums, in
order to assist comprehension. Following admission, the placement of each individual is reviewed at least twice yearly and comprehensive documents about their progress, including a risk assessment are provided to them and their referring agency.
