Criteria for Admission

Provision
is for male and female adults aged 16 years and over.

In
addition to hearing loss, individuals may experience additional compound and
complex disabilities, for example Physical/Learning Disabilities, Visual
Impairment, Mental Illness and Autism.
Those with challenging behaviour will also be considered.

Where
applicants demonstrate challenging behaviour, the physical and emotional
welfare of other service users and staff will be considered during the
selection process.

At
present Hayfield offers limited access for wheel chair users.

Applicants
with acknowledged drug or alcohol problems would only be accepted into
Hayfield if it were considered that the prognosis for them ceasing their
addiction was strong.
The Process
-
Applications
are made by social workers on behalf of the user.

The
admission panel will review the information and assess the needs and
suitability of the applicant.

The
pack, consisting of appropriate background information, medical report and
financial agreement should be completed and submitted to Hayfield for each
applicant.

A
six week Screening Evaluation Period provides the applicant and the
Screening Panel the opportunity to assess the appropriateness of the
placement.

The
applicant will be invited to undertake a six-week Assessment and Evaluation
prior to any final decision being made about a placement in Hayfield.

At
the end of this period, an Admission Assessment Meeting will be held at
which the applicant, their family/carers and representatives of the
referring authority will meet with Hayfield staff to discuss and decide on
the appropriateness of a future placement in Hayfield.

Evaluation
reports will be produced and made available to the applicant and referring
agency.

Following
admission, the placement of each individual will be reviewed at least twice
yearly.
