Job Description - Social Worker

 

Main Function

To provide a Social Work service to identified individuals, within the framework of Hayfield policies and procedures and relevant legislation.

 

Core Duties

  • To provide appropriate counselling, advice, guidance and forms of intervention to service users within Hayfield, but also to carers and staff as appropriate.

  • To be part of the team establishing the individual needs assessment, care planning and care management service and to ensure regular monitoring and review of same.

  • To work in partnership with services users, Day Care and Residential services staff, to assess and identify service user needs and facilitate service provision which is outcome focused and enhances quality of life, independence and social integration.

  • To liaise with appropriate agencies on behalf of service users.

  • To record all work undertaken in accordance with Hayfield procedural instructions.

  • To participate in formal consultation/supervision sessions as required by Hayfield policy and critically analyse personal practice and performance in conjunction with line management.

  • To work where appropriate with other disciplines or in multi-disciplinary settings to research and develop best practice in the areas of deafness, mental health, learning disability, autism and complex disability.

  • To participate in Reviews, including involvement in the identification of service user current need and the development and improvement of services.

  • To undertake such training programmes as may assist personal and continuous professional development and organisational development.

  • To widely promote public understanding of the services of Hayfield, both informally, via presentations and by attendance at exhibitions.

 

Additional Duties

  • To participate in conferences and seminars specifically to assist Hayfield to remain in the forefront of specialised work with Deaf people.

  • To establish links and network with other organisations/authorities working with Deaf people in Education, Psychology, Psychiatry, Health Care and Community Care.

  • To remain updated in social care developments, best practice, policy, resources and legislation relevant to Hayfield service users.

  • To work with the Management Team and Director, as appropriate and undertake specific responsibilities on behalf of Hayfield as identified by line management.

  • To supervise students/trainees, as appropriate.

  • To undertake other duties and responsibilities as directed by line management.

  • There is an expectation that Hayfield employees deliver and promote the positive benefits of equality and diversity in the manner in which they carry out their duties and responsibilities.

  • All Hayfield employees are expected to adhere to the Health and Safety Policies and promote and maintain a safe environment.

  • All Hayfield employees are expected to adhere to the Scottish Social Services Council Codes of Practice.
