Job Description - One-to-One Support Worker
Accountable To
Appropriate
Manager
Main Purpose of Post
Working in
various locations on a one-to-one basis with a service user to provide mental
stimulation and offer general support and further enhancement of quality of
life.
After
appropriate induction training; support and care for one service user in line
with Hayfield policies and strategies.
Key Responsibilities

Ensure
appropriate personal care and support for service user, retaining the
principles of dignity, privacy and respect at all times.

Be
involved in developing programmes of activities appropriate to service user
needs, health and well-being.

Be
involved in developing effective communication systems with service user.

Contribute to developing Care Plans/Work Development Plans, following
Goal/Target Plans and attend reviews.

Liaison
with others about daily outcomes and ongoing developments.

Availability to work shifts (day, evening and weekends).
-
Carry out
any other reasonable duties that may be required.
N.B. You
will be expected to familiarise yourself with Hayfield Policies and Procedures,
including Health & Safety matters and Confidentiality aspects.
